Application Process
The application process takes an average of 8 to 10 weeks, from the time we receive a completed Application Form to the time your file is complete and ready for review by our Admissions Committee.
Here’s how it works:
Step 1. The Application and Release of Information Form
The application process officially begins when we receive your completed Application and Release of Information forms. Receipt of your completed application places you tentatively (pending approval) on our six-month waiting list for class.
Submit these forms by mail or through our on-line application process. Pay particular attention to details and do not leave any blanks.
Step 2. References
We will send letters to each of the people you cite as references on your Application form, requesting information about you.
These people should include an Orientation and Mobility Instructor, a Counselor and 3 Personal References. Personal References cannot be family members and must be different from your O&M Instructor and Counselor.
Step 3. Medical Forms
Submit the Guiding Eyes Medical Form to your physician for completion.
Ask your primary physician to send or fax the form directly to Guiding Eyes.
Step 4. The Home Interview
A Field Representative will phone you shortly after you begin the application process to arrange an in-person interview with you at your home. We will assess your travel needs and readiness for Guiding Eyes dog training.
The Home Interview gives you an opportunity to learn more about our training program.
Step 5. Admissions Committee Review
Once all of the above steps of the application process are complete, our Admissions Committee thoroughly reviews your file. We will promptly inform you of the Committee’s decision.
