Apply for Guiding Eyes training online by using the form below. The form below is for returning Guiding Eyes graduates only. If you have applied to Guiding Eyes before but not graduated or you are a graduate of another school, please use the First Time Applicant Form. Please note that these applications must be completed in full and submitted. When working online you are not able to save unfinished applications.
