The following Health and Work report will provide valuable information needed by Guiding Eyes for the Blind to ensure the health of our future guide dogs. This information is used by both the Veterinary Hospital and the Canine Development Center to track and help eliminate future problems such as ear infections, allergies and stomach issues that may sometimes occur with our dogs. The information you provide helps ensure a longer working life for our guides. Thank you for your cooperation in filling out this report.

Please note the eligibility requirements for “Transfer of Title”.

It is dependent upon yearly submission of the Health & Work Report, the graduate being of good standing and your guide dog must be within 10 pounds of his or her target weight. It is the graduate’s responsibility to request Transfer of Title.
