Volunteer Application Forms

Thank you for considering volunteering with Guiding Eyes. Here you’ll find all our application forms in one easy-to-find place. Every one of our opportunities is different, so to make sure you find the best fit, please take the opportunity to read the detailed information about the program you are interested in. Learn more about volunteering with us, prior to submitting an application.

All volunteers are asked to review and adhere to a Code of Conduct.

Puppy Raising

If you have reviewed the Puppy Raising information, including the Puppy Raiser Checklist, and would like to proceed to the next step, please submit the Puppy Raiser Application. We will email your application to the region nearest to where you reside. You will hear from a Guiding Eyes representative within two weeks and will be invited to attend classes that will answer your questions and help you prepare to become a puppy raiser. We ask our potential raisers to be well informed in order to make this important decision.

For additional information, please contact us at cdc@guidingeyes.org.
