Volunteer Application Forms

Thank you for considering volunteering with Guiding Eyes. Here you’ll find all our application forms in one easy-to-find place. Every one of our opportunities is different, so to make sure you find the best fit, please take the opportunity to read the detailed information about the program you are interested in. Learn more about volunteering with us, prior to submitting an application.

All volunteers are asked to review and adhere to a Code of Conduct.

Puppy Raising

If you have reviewed the Puppy Raising information, including the Puppy Raiser Regions and Raiser Checklist, and would like to proceed to the next step, please sign up for an upcoming virtual information session. These sessions are hosted by our Regional Puppy Instructors and will provide an overview of puppy raising, help to answer your questions and help you prepare to become a puppy raiser or puppy sitter. We ask our potential raisers to be well informed in order to make this important decision.

For additional information, please contact us at cdc@guidingeyes.org.

Brood/Stud Foster Program

If you have reviewed the Brood Stud Program information and are interested in becoming a Foster, please submit this application for consideration. The Brood/Stud Manager will contact you to discuss your application in greater detail. Our standard response time is 7-10 business days.
